54 Forward Hoops (“54FH”, “we”, “us”, “our”) is a basketball development program led by Coach Danny, operating in St. Albert and Edmonton, Alberta, Canada. This Privacy Policy explains what personal information we collect, why we collect it, how we use and protect it, and the choices you have.
By using our website or submitting our registration form, you agree to this policy. Because our athletes are minors, we collect information from parents and guardians and treat it with particular care.

2. Information We Collect
Athlete information
- Full name, date of birth, age, gender
- School and current grade
- Basketball experience, playing position, skills to improve, and goals
Parent/guardian information
- Full name and relationship to the athlete
- Phone number and email address
- Preferred method of communication
Emergency contact information
- Name, relationship, and phone number
Health and safety information (sensitive)
- Injuries or physical limitations, allergies, asthma, and other medical conditions or health concerns
- Special accommodations needed during training
- Whether the athlete carries emergency medication (for example, an inhaler or EpiPen)
- Any additional medical notes you choose to share
Other information
- Training location and attendance frequency preferences
- Photo and video permission choice
- How you heard about us, and any comments you provide
- Attendance confirmations you send us by text message
- Payment-related information needed to confirm participation, e.g., e-transfer records; we do not store full card numbers
- Basic technical data from our website (such as browser type and pages visited), if analytics or cookies are used
3. How We Use Your Information
We use personal information to:
- Register athletes and manage participation in training sessions
- Communicate about schedules, attendance, updates, and important announcements
- Plan sessions and tailor training to each athlete’s level and goals
- Protect athlete safety, including responding to medical emergencies
- Contact parents/guardians and emergency contacts when needed
- Process and track payments
- Promote 54FH through photos and videos, only where permission has been given
- Understand how families find us and improve our program and website
- Meet legal obligations
4. Consent
We collect, use, and disclose personal information with a parent’s or legal guardian’s consent. By submitting the registration form, the parent/guardian confirms they have authority to provide the athlete’s information and consents to its use as described here.
You may withdraw consent at any time (see Section 10), though withdrawing may limit our ability to let the athlete participate safely.
5. Health Information
Medical information is collected solely so our coaches can keep athletes safe. It is shared only with coaches and staff who need it, and with emergency responders or medical professionals if an emergency occurs. We do not use health information for marketing or any unrelated purpose.
6. Photos and Videos
Photos and videos may be taken during training. Our registration form asks parents/guardians whether 54FH may use them for promotional purposes (social media, our website, marketing, and academy content).
- If permission is given, we may publish images of the athlete in those channels. We will not publish an athlete’s full name alongside an image without separate consent.
- If permission is not given, we will not use identifiable images of the athlete for promotion, and will make reasonable efforts to keep them out of published content.
- Permission can be withdrawn at any time by contacting us. We will stop future use and remove content we control where reasonably possible, but we cannot recall content already shared or downloaded by others.
7. Sharing Your Information
We do not sell personal information. We share it only as follows:
- Service providers: Tools we use to run 54FH, such as Google (Google Forms, Sheets, Drive, and email), our website host (Hostinger), messaging or phone providers, and payment methods. They may process information on our behalf and are expected to protect it.
- Facility partners: Where required by a venue we train at (for example, Legacy Athletics), limited information such as attendance or emergency details may be shared.
- Emergencies: With emergency services and medical professionals if an athlete needs urgent care.
- Legal reasons: Where required by law, court order, or to protect rights, safety, or property.
- Business changes: If 54FH is reorganized or transferred, information may be passed on to the successor, who will be bound by this policy.
8. Storage, Location, and Security
Registration information is collected through Google Forms and stored in Google’s systems, and our website is hosted by Hostinger. As a result, information may be stored or processed outside Alberta or Canada, including in the United States, where it may be subject to the laws of those jurisdictions.
We use reasonable safeguards, such as restricted access and account protections, to guard information against loss, misuse, and unauthorized access. No online system is completely secure, so we cannot guarantee absolute security. Please do not send passwords or payment card details through our forms or text messages.
9. Retention
We keep information only as long as needed for the purposes above or as required by law. Registration and medical records are retained for [e.g., the duration of participation plus 2 years] and then securely deleted or anonymized. You may request earlier deletion (see Section 10).
10. Your Rights and Choices
You may contact us to:
- Access the personal information we hold about you or your athlete
- Correct information that is inaccurate or out of date
- Withdraw consent, including photo/video permission
- Request deletion of your information, subject to legal requirements
- Opt out of non-essential communications
- Ask questions or raise a privacy concern
We will respond within a reasonable time, generally within 30 days. If you are not satisfied, you may contact the Office of the Information and Privacy Commissioner of Alberta (oipc.ab.ca) or, where applicable, the Office of the Privacy Commissioner of Canada (priv.gc.ca).
11. Children’s Privacy
Our program serves children and youth. We collect information about athletes only from their parents or guardians and do not knowingly collect information directly from children without parental involvement. If you believe a child has provided us with information directly, please contact us, and we will delete it.
12. Website, Cookies, and Third-Party Links
Our website may use cookies or similar technologies to operate properly and understand usage. You can control cookies in your browser settings. Our website and social media pages (Instagram, Facebook) may link to third-party sites whose privacy practices we do not control. Please review their policies.
13. Communications
We contact families by text, email, or phone according to the preference selected on the form. These messages relate to training and program operations. Standard messaging rates from your carrier may apply.
14. Changes to This Policy
We may update this policy from time to time. The “Last updated” date above shows the latest version, and material changes will be communicated through our website or directly to registered families.
